REQUESTING A TOP DOC MED NOTE IS EASY!

1. SUBMIT YOUR FORM: Click HERE to submit your Top Doc Med Note Request Form. Please include any information needed to create your HIPAA-compliant notes and upload supporting documentation. Please review sample notes below!
   
2. PAY YOUR INVOICE:  Check your email! We will email you an invoice for your Top Doc Med Note(s) generally within 24-48 hours.
   
3. RECEIVE YOUR NOTE:  You will receive an email when it's ready! You will need to enter the secure access code delivered to you via text or phone call to access your note. 
   
4. SAVE AND PRINT YOUR NOTE:  When your note open, click "Print" in the right upper corner.  This will open your note in PDF format.  You will be able to download and print your note from here.
